Dog Vaccination Form by Laryssa Hook — last modified 2008-08-07 13:56: This form is required for the Ohio State Fair and Delaware County Fair. If you participate in the county work out dates, you will also need this form or other proof (vet. receipt) of the necessary vaccinations.
